To complete your application, you'll need some basic information and documents that provide proof of residency: Full legal name, address (current or permanent), phone number, date of birth, sex/gender identification. You will also need to include a photo ID with a signature such as a driver's license or passport. And finally, you'll have to submit two pieces of paper showing everyone who lives at your residence with their full legal names, addresses(current or permanent), and signatures on both sheets! This includes every member 18 years old and over including parents if they live there too!

How long does it take before getting approval?

After you submit your application, it takes at least 45 days for us to process the request and issue a license. The processing time may be longer if you provide incomplete information or send in documents that are not clear copies. If all required materials have been submitted correctly, we will typically process an application within 15 business days of receiving the final packet from applicants. We'll let you know when your MMAR Medical Marijuana License has been approved!

How much does it cost?

The fee is $50-$200 depending on category type: Individual Resident Patient with Serious Health Condition- $150; Non-Profit Organization -$500; Qualifying Person (includes caregivers) under 18 years old with Serious Health Condition- Free*; *plus a $60 application fee, if 18 years old or older.

The MMAR Medical Marijuana License is not required for patients who are registered with the OMMP (Oregon Medical Marijuana Program). The license will expire one year after it's issued by our department to any qualifying patient that does not have an active registration in the Oregon Medical Marijuana Program. Please follow the instructions on filling out application form and be sure to include a photo ID with date of birth, proof of address (utility bill, lease agreement). Application fee is $200 for first-time applicants or renewals.
